  • PASSHE Distance Education Application
    Apply to take a distance education course from another PA State System (PASSHE) institution. (Requires Transfer Credit Equivalency/Evaluation.)
  • PASSHE Visiting Student Form for Graduate Students
    Form permits graduate students to apply to take classes at another Pennsylvania State System of Higher Education (PASSHE) institution. Registering as a visiting student will permit courses to be transferred across the System, without loss of institutional residency, eligibility for honors or athletics, or credit towards graduation at the home institution. Students do not need to complete a separate Transfer Credit Permission form.
  • PASSHE Visiting Student Form for Undergraduate Students

    Request re-evaluation of transfer credit.

    Obtain approval to transfer credit as part of an international education experience.
  • National Student Exchange Program Approval
    All students planning to take courses while on exchange must complete the form and return it to the NSE Coordinator in the Center for International Programs (Mitchell Hall, 3rd Floor).
  • Reverse Transfer Release Form
