Please complete this form to the best of your ability and provide as must detail as possible regarding the incident and any resulting damage.
Photos of all damage should be taken and emailed to autoshop@wcupa.edu as soon as possible, along with a completed STD-541 Accident/Loss Notice. The STD-541 form is located in the Accident Report Packet in the glove box of every University vehicle.
Reminder: If the incident involves another person's vehicle, property, or injury, a police report should be filed.
Motor Pool staff will review the information provided and will contact you if any additional documentation or forms are required.
