Instructions
The Sexual Misconduct Incident Report Form can be filled out by a student,
staff, faculty, community member, parent, or anyone with information to report
about an act of sexual misconduct involving a WCU employee or student. Sexual
misconduct includes sexual harassment, sexual assault, sexual exploitation,
and stalking.
This form can be used to file a formal, informal, or anonymous report.
Those who have been identified as Campus Security Authorities (CSAs) should
complete both the CSA form and the Sexual Misconduct Report Form. CSAs should
start by completing a Clery Act Report found here:
www.wcupa.edu/csa. You will then be
prompted to complete a Sexual Misconduct Report Form in another window, once the CSA form
is complete. Please make sure you complete both forms as a CSA. If you have any
questions about Campus Security Authorities (CSAs) or the Clery Act Report,

The information that you include in the following report will be shared with
the following offices. If you would prefer to speak with someone in person or
by phone, you may contact the office directly at the telephone numbers or
addresses below. If you indicate on the form that you want to be contacted
by one or more of these offices on the form, a representative from that
office(s) will contact you at the telephone number or email address that
you provide.
- To file a police Report: Those who want to press criminal charges
against a perpetrator(s) should contact the Department of Public Safety
(610-436-3311), dial 911, or contact the police in the area where the
assault took place. Filing a report with the police is necessary when
a victim would like to pursue a criminal investigation by the police
and courts.
- To file a WCU Judicial Report: Sexual misconduct is a violation of
the Student Code of Conduct, and therefore a victim may make a judicial
complaint against a WCU student(s). A judicial investigation and hearing
is separate from a criminal investigation by the police and courts.

- To file a complaint for Title IX violation: Sexual harassment, which
includes acts of sexual violence, is a form of sex discrimination
prohibited by Title IX of the Education Amendments of 1972. A victim of
any form of sex discrimination, sexual violence, or gender-based harassment
may file a complaint with the WCU Title IX coordinator: Barbara Schneller,
