Questions
Use these as references for you throughout your CCP
experience
Entry Form
Complete the entry record form and save your information
at the conclusion of each semester. You only have to
enter information for the current semester. However, if
additional activities from past experiences have not
been documented, then please include these in your
entry. When you are ready, email the entry form to
Portfolio@wcupa.edu
or drop them off at 238 Sykes or the Leadership Resource
Center.

Personal Reflection
At the conclusion of each semester, answer the personal
reflection questions about your co-curricular activities.
The ASK Model and personal descriptors can be used to guide you through this
process.
If this is your first time working with the
Co-Curricular Portfolio and you have been at WCU for
more than 2 semesters, please reflect on your entire
co-curricular experiences to date. For example, if
you are a junior and this is your first time working
with the Co-Curricular Portfolio, please reflect on
your first 2 years at WCU in one personal reflection.

Request Form
Finally, if you need to request
copies of your CCP before the end of each academic
year, please complete the request form and submit it
to Portfolio@wcupa.edu
so that an official portfolio can be compiled
for you. A request may take up to 14 days to be
completed. An email notification will be sent to
you when your CCP is available for pickup at either
238 Sykes or the Graduate Assistant's office.
