Below is a library of helpful information and commonly used forms available for your use. For your convenience you may fill in these forms electronically, or print, complete, and submit, if required. Please read the instructions carefully and submit with any and all required documentation (when appropriate) to the Financial Aid Office. Each form includes a description of its purpose.

Academic Development Program (ADP)

Athletic Aid

  • 2015-2016 Athletic Grant-In-Aid Agreement : NCAA Required form for all athletic awards. Student Athletes can complete, sign and date this form, and return to Dan McIlhenny, dmcilhenny@wcupa.edu. This award is contingent on the recipient’s admission to the university and upon agreement to comply with NCAA and University regulations and policies governing a Grant-In-Aid.

FAFSA: Free Application for Federal Student Aid

  • FAFSA Apply Now: Application used to apply for Federal and State Student Assistance. Priority deadline is March 1st. Must be filed each year. Our Federal code is 003328.


  • Financial Aid Shopping Sheet : The "Shopping Sheet" is a consumer tool used to notify students about their financial aid package. It is a standardized form that is designed to simplify the information that prospective students receive about costs and financial aid so that they can easily compare institutions and make informed decisions about where to attend school.

Federal Work-Study (FWS) Student Employment

  • Federal Work-Study Request to Transfer Form : Used to request the transfer of a FWS employee to another department. May be initiated by either the student or the supervisor. Must contain both parties' signatures. Transfers are not guaranteed and reassignment is based on the vacancies available at the time of request.


  • Federal Work Study Student Employment 2016-2017 Application : The WCU Federal Work-Study Student Employment Application is available January 1st. The priority deadline is March 1st.

    To apply for Federal Work-Study (FWS) - complete your FAFSA AND complete a West Chester University Student Employment Application as early as possible but in compliance with each application's priority deadline.

    Students offered federal work study may not hold another on campus job processed by the WCU Payroll office, while participating in the FWS Program.


  • Federal Work Study Summer Student Employment WCU Application: Funding for the summer is limited. Eligibility will be determined after Spring grades are posted and academic progress has been determined.

    Eligible students will receive a contract via their WCU email account.

    Submission of the application does not guarantee a summer employment position.

    Priority Deadline: May 1st.


  • Federal Work Study Student Employment Handbook : Handbook and guidelines for Students and Supervisors.


  • Federal Work-Study- Job Descriptions : Search the job descriptions for positions available.


  • Confidentiality Statement Form : Departments that engage Federal Work Study Students in work that involves confidential and information, documents and materials are required to collect a confidential statement from each of their students. This document should be forwarded to the financial aid office.


  • Direct Deposit Form : Used to authorize wages earned to be directly deposited into student's bank account.

    All students are encouraged to take advantage of this service for the prompt delivery of payment.

    1. Print form
    2. Fill out the form completely
    3. Write "void" on a personal check
    4. Take the form & your voided check to the Human Resources/Payroll Office.

Grants

  • Enrollment Change Form - PA Residents : Used to request the PA State grant when West Chester University was not listed as the 1st choice on the FAFSA.


  • PHEAA PA State Grant Forms at PHEAA.org : Forms made available by the Pennsylvania State Grant Agency such as enrollment change forms, incomplete status notice, special circumstance forms etc.


  • Summer 2015 PHEAA State Grant Application - Available in the spring at PHEAA.org: Summer state grants are available to those who qualify. Students must be enrolled for two of three summer sessions minimum with a combined credit load of at least six (6) credits. Students may not be eligible for a summer PA grant if more than 50% of the summer enrollment is comprised of distance education courses.


  • PHEAA State Work Study Program Application and Placement Form at PHEAA.org : The State Work-Study Program (SWSP) provides Pennsylvania students with employment opportunities in high-technology and community-service fields.


  • TEACH Grant Request Form: This form is used to request a TEACH Grant once the student has been granted formal admission to Teacher Education Program, in one of the eligible programs of study.

Loans

Satisfactory Academic Progress (SAP)

  • Satisfactory Academic Progress Appeal
  • This application is used to request reinstatement of your federal financial aid when you have been notified by the aid office that you are in noncompliance of the Satisfactory Academic Progress policy.

    The appeal process requires that students meet with their academic advisor and provide a copy of an Academic Recovery Plan/Academic Success Plan. Students granted an APPEAL will be placed on Financial Aid Probation (FAP) and will receive Title IV- Federal Aid for one (1) pay period/term. At the end of the probation period the student's academic progress will be reviewed again. Submission of this application does not guarantee reinstatement of canceled Federal IV financial aid. Students should discuss alternative payment options with the Bursar's office.

    Students are encouraged to discuss their concerns and seek guidance and help from the Department of Counseling & Psychological Services and the WCU Health and Wellness Center, where it is appropriate.

    Appeal Deadlines:

    Appeals received after the deadline are subject to decline and may be returned to the applicant without committee action.

    Fall or Spring submit at least 3 weeks BEFORE the semester begins.

    Summer should be submitted 2 weeks BEFORE prior to the start of the summer session.

Special Situations

  • Appeal To Be An Independent Student Request Form 2014-15 | Appeal To Be An Independent Student Request Form 2015-16 : Parental information is required for students who are not independent according to federal statute. Appeals will be considered after the student has:

    1. Had a telephone or in-person appointment with the appropriate Financial Aid Staff and
    2. All required documentation of extenuating family circumstances has been provided.

    Please do not complete and submit any documents until after your appointment with FA Staff. Please carefully complete Step # 3 of the FAFSA. For added insight please see our FAQs concerning this topic.

    The decision of the aid office is final and cannot be appealed.

  • Unaccompanied/Homeless Youth (UHY) Determination Form : To verify students who have answered "yes" to being an unaccompanied youth who was homeless or at risk of being homeless on the Free Application for Federal Student Aid (FAFSA).

  • Unusual Circumstance/Income Reduction Worksheet: Available by Request ONLY

    Undergraduate students, with a "zero" EFC need not submit the "U. C." worksheet.

    This "U.C." worksheet is used to report both reductions to income after the FAFSA was completed and unusual circumstances that affect your family's ability to contribute money to the cost of your education i.e. high medical expenses, dependent care, unusual debts, etc. For added insight please see our FAQs concerning this topic.

    Your Unusual Circumstances request will NOT be processed until federal verification is complete.

    Parents or students that submit the "U.C." worksheet will also be selected for "federal verification" and must submit the appropriate verification worksheet with any/all requested materials. Please submit both the verification worksheet and the unusual circumstances/income reduction worksheet together in one packet.

Student Records

  • Authorization to Release Information Form : This form may be used to authorize or restrict the release of information pertaining to an individual student's West Chester University records.


  • Consortium Agreement Form : This form serves as an agreement between the student's Home school and Host school where the student may be earning credits. Also available in the financial aid office.


  • Financial Aid Notice - FAN : Letter containing the student's financial aid awards.

Verification

Most of the forms with a pdf icon () can be filled out electronically using Adobe Reader and then printed. Please note where signatures are required.

Veterans

  • Selective Service Status Information Letter at SSS.gov : There may be a time when a man declares that he is not registered with the Selective Service System and is applying for federal student assistance such as loans, grants and employment. As such, a Status Information Letter may be required by the financial aid office. For additional information please see link provided.

  • Veteran's Benefit Request for Certification Form : (Summer 2016 & Academic Year 2016-2017)

    This form will along with all other required documents are required for certification of VA Educational Benefits. This form is for the Summer 2016, and the Academic Year 2016-2017. For additional information and assistance please visit the Veterans Center located at 624 S. High Street.
