Frequently Requested Forms

Below is a list of commonly used forms that you can download, print, complete and submit. Each one includes a description of its purpose. Please read the instructions carefully and submit with any and all required documentation (when appropriate) to the Financial Aid Office.

Appeal Deadlines:

This application is used to request reinstatement of your federal financial aid when you have been notified by the aid office that you are in non compliance of the Satisfactory Academic Progress policy. The appeal process requires that students meet with their academic advisor and provide a copy of an Academic Recovery Plan/Academic Success Plan. Students granted an APPEAL will be placed on Financial Aid Probation(FAP) and will receive Title IV- Federal Aid for one (1) pay period/term. At the end of the probation period the student's academic progress will be reviewed again. Submission of this application does not guarantee reinstatement of canceled Federal IV financial aid. Students should discuss alternative payment options with the Bursar's office.

Students are encouraged to discuss their concerns and seek guidance and help from the Department of Counseling & Psychological Services and the WCU Heath and Wellness Center,  where it is appropriate.

Appeals received after the deadline are subject to decline and may be returned to the applicant without committee action.

Applications for the Summer should be submitted 2 weeks prior to the start of the summer session.

Appeals for an academic semester should be submitted at least 3 weeks BEFORE the semester begins.

Please use the correct application for the time period you are appealing.

To apply for aid for the summer 2013 you must complete the 2012-2013 FAFSA and the Summer Financial Aid Application. To apply for aid for 2013-2014 FAFSA, we encourage you to apply by the March 1st priority deadline. For additional information please review the attached ADP Brochure. Parental information is required for students who are not independent according to federal statute. Appeals will be considered after the student has 1.) had a telephone or in-person appointment with the appropriate Financial Aid Staff and 2.) all required documentation of extenuating family circumstances has been provided. Please do not complete and submit any documents until after your appointment with FA Staff. Please carefully complete Step # 3 of the FAFSA. The decision of the aid office is final and cannot be appealed.

 

Authorization to Release Information Form This form may be used to authorize or restrict the release of information pertaining to an individual student's West Chester University records.

Consortium Agreement

This form serves as an agreement between the student’s Home school and Host school where the student may be earning credits. Available in the financial aid office.

Federal Student Aid (FAFSA)

Application used to apply for Federal and State Student Assistance.  Priority deadline is March 1st.  Must be filed each year.  Our Federal code 003328.

PA State Grant forms

Forms made available by the Pennsylvania State Grant Agency such as enrollment change forms, incomplete status notice, special circumstance forms etc.

PHEAA State Work Study Program

The State Work-Study Program (SWSP) provides Pennsylvania students with employment opportunities in high-technology and community-service fields. Work-Study Times and SWSP Application and Placement Form.

2013-2014 Federal Work Study Student Employment Application

The priority deadline is March 1st.

To apply for Federal Work study (FWS) - complete your (FAFSA)  AND complete a West Chester University Student Employment Application by

the March 1st priority deadline. Depending on the volume of applications applications are subject to being wait listed.

Students offered federal work study may not hold another on campus job, processed by the WCU Payroll office, while participating in the FWS Program.

Selective Service Status Information Letter

There may be a time when a man declares that he is not registered with the Selective Service System and is applying for federal student assistance such as loans,     

grants and employment. As such a Status Information Letter may be required by the financial aid office. For additional information please visit http://www.sss.gov/Status.html. see link provided.

Summer 2013 Loan Request

Priority deadline for completing/submitting the request April 15th; deadline for submission is JULY 1st.

Summer sessions are considered "trailer" at WCU. Summer funds are limited. Submission of the application does not guarantee aid for the summer.

Please pay close attention to the instructions and information provided on the application. The Summer 2013 Loan Request. Available on-line only.

Federal Work Study Student Employment Application

Priority Deadline: 

June 14th

 

Funding for the summer is limited.

Eligibility will be determined after Spring grades are posted and academic progress has been determined.

Eligible students will receive a contract via their WCU email account.

Submission of the application does not guarantee a summer employment position. (Available in aid office only.)

 

Summer 2013 PHEAA State Grant Application

Summer state grants are available to those who qualify. Students must be enrolled for two of three summer sessions minimum with a combined credit load of at least six (6) credits. Summer state grant applications are available at www.pheaa.org.

 

Teach Request Form

Summer 2013 TEACH Grant Request Form

This form is used to request a TEACH Grant once the student has been granted formal admission to Teacher Education Program, in one of the eligible programs of study.

The 2012-2013

Unusual Circumstance/Income Reduction Worksheet.

 

This "U.C." worksheet is used to report both reductions to income after the FAFSA was completed and unusual circumstances that affect your family’s ability to contribute money to the cost of your education i.e. high medical expenses, dependent care, unusual debts, etc. For added insight please see our FAQs concerning this topic.

Please make an appointment (in person or by phone) with Mr. Foultz  before completing and submitting any Unusual Circumstances documents. Please contact the

FA Office (610) 436-2627 to make an appointment.

Your Unusual Circumstances request will NOT be processed until federal verification is complete.

Parents or students that submit the "U.C." worksheet will also be selected for "federal verification" and must submit the appropriate verification worksheet with any/all requested materials.  Please submit both the verification worksheet and the unusual circumstances/income reduction worksheet together in one packet.

Undergraduate students, if your EFC is already “zero” do NOT submit the “U. C.” worksheet as you will already be receiving the maximum financial aid that you can receive.

Graduate Students are limited by federal law to Unsubsidized Loans Only beginning in the 2012-2013 Academic year.  Since subsidized graduate student loans are no longer available to graduate students WCU will not accept UC appeals for that purpose.

Request Tax Return Transcript at IRS.gov

The form contains instructions as to how to obtain a copy of one’s federal income tax (1040) form for submission in compliance with the verification process .
