CAPC – Forms, Handbooks, and Rubrics

Below you’ll find the latest versions of all CAPC forms, as well as the rubrics used to evaluate all new courses, programs, and all modifications to courses and programs.  Please complete the forms electronically, utilizing the checkboxes in place of signatures and forwarding the forms on as necessary.  The documents can be saved before forwarding to the Associate Provost’s Office for CAPC review.

Please make sure that each form is accompanied by the appropriate document – syllabus, other CAPC forms as necessary (for I, W, and J approvals), etc.  Completed forms should then be sent to Andrea Grinwis ( agrinwis@wcupa.edu).

 

Forms

Instructions:  The most often utilized forms are found directly below.  Simply click on the form that best suits the needs of your operating system.  Also, be sure to review the rubrics below. All syllabi will be reviewed using those rubrics.

 

  • Course Approval or Modification Form
    • Mac Users: This form is a fill-in Word document that can be saved to your computer before forwarding on for signatures.  You will need Microsoft Word to use the form.  If you have a university-purchased Mac, the IT Help Desk will install Word on your laptop for free.  If your Mac was purchased for personal use, you will need to (a) use your university-provided PC, (b) purchase Word, or  (c) use Google Docs.  The form will not work in Text Edit.

 

  • Program Action Form
