Incoming Students

Record Requirements

  • In order to provide high quality health services, all incoming students must complete a Student Health Form and Immunization Record.
  • The medical history and immunization record should be completed and signed by the student.

  • Send health information by the beginning of the semester to avoid a temporary hold on the student account, which prevents future class registration and release of grades.
