Mailing Request Forms - {Blue Card Replacement}

In order to efficiently process University mailings, the University Post Office utilizes a bar code scanner to input the Cost Center or WBS Element to be charged for outgoing mail. These web forms will generate a printable replacement for the previously utilized "blue card". Select the appropriate form for the source of funds.
