Purchasing Card Enrollment

Utilize this form to enroll in the Purchasing Card program. Fill in all of the fields and then click the Prepare Form button at the bottom of the page. The information that you enter will be used to prepare the enrollment document as you click the "Prepare Form" button. You then just click the "Print the Form" button located in the top right corner of the document. Use the printed form to obtain the required signatures and send the completed document to Sandy English in Business Services (201 Carter Drive) for processing.

Prepare a form for each purchasing card you need. All fields are required. If you are applying for a travel card please utilize the Travel Card Enrollment form.
